The American Hearth Association recently published an updated, thoroughly referenced, scientific statement about caffeine and its POSITIVE EFFECTS on cardiovascular health.

Caffeine, imbibed as coffee, is the most commonly consumed beverage in the US, and probably Europe, too.

Caffeine affects multiple organs and we all metabolize caffeine at different rates (depending on inherited genetic variants and prior patterns of exposure).

More than sixty plant species, including cacao beans and tea leaves, naturally contain caffeine. It is also found in many over-the-counter and prescription products. Caffeine is 100% bioavailable, meaning it rapidly dissolves and is absorbed on consumption. Increasing doses of caffeine, i.e. more cups of coffee, result in proportionately increasing blood levels.

Caffeine is metabolized by liver enzymes and excreted into the urine.

Caution: the composition of energy drinks, through the presence of ingredients such as taurine, increase the solubility, absorption and onset of action of caffeine.

There is an inverse association between coffee intake and risk for type 2 diabetes and cardiovascular disease. It has been suggested that bioactive compounds in coffee have antioxidant and anti-inflammatory properties and may improve insulin sensitivity.

There is an inverse J-shaped association of habitual coffee consumption with the risk of hypertension: increased risk with 1-3 cups/day, but a trend towards decreased risk with higher intake.

There is a twenty to thirty percent decreased risk of type 2 diabetes among people who, on average, consume 3.5 – 5 cups/day. Coffee contains chlorogenic acid, a polyphenol that improves glucose metabolism and 2 micronutrients, magnesium and chromium, both linked to insulin sensitivity and lower blood glucose.

Caffeine and caffeinated coffee consumption is associated with no difference in risk or a lower risk for atrial fibrillation. Similarly, there is no relationship between caffeine when consumed in coffee and tea and premature atrial contractions (PACs).

In a meta-analysis of thirty prospective cohort studies, consumption of up to 6 cups of coffee per day was NOT associated with a higher risk of coronary artery disease (CAD). Moreover, light (1.5 cups/day) and moderate (3.5 cups/day) were associated with a ten percent lower risk of CAD.

In addition, coffee consumption is not associated with a higher risk of cardiovascular disease or mortality in individuals with a history of myocardial infarction (a heart attack).

There is a twenty percent risk reduction in stroke associated with moderate coffee consumption (3-4 cups/day). There is a modest inverse association between tea consumption and stroke risk, with similar association for black and green tea.

Moderate caffeine or coffee consumption - up to 400 mg/day caffeine or 3-5 cups of coffee/day - is SAFE for most adults and is associated with lower risk of cardiovascular disease, including CAD, stroke, heart failure, atrial fibrillation, and hypertension, as well as type 2 diabetes.

There are modest inverse associations between habitual coffee drinking and all-cause mortality across diverse populations. Similar inverse associations have been reported for both caffeinated and decaffeinated coffee, supporting the contribution of noncaffeine bioactive components.

Habitual moderate coffee consumption can be part of a healthy lifestyle. Although coffee is the main source of caffeine for most adults, tea, chocolate and energy drinks also contribute to overall caffeine intake.

Acute effects of caffeine can include transient increase in bold pressure, blood sugar, and alertness, as well as palpitations and sleep disruption in some people.

Caution: potential health benefits observed among individuals consuming caffeine to occur in natural substances should NOT be extrapolated to synthetic products or high-dose caffeine. Although data on energy drinks are limited, available studies suggest cardiovascular HARM.

Energy Drink Warnings: Highly caffeinated energy drinks and concentrated shots lack the benefits of natural coffee. Because they can contain three to four times the caffeine per ounce, they are linked to blood pressure spikes and life-threatening heart arrhythmias.

The American Academy of Pediatrics (AAP) advises adolescents (ages 12-18) to consume no more than 100 mg of caffeine daily. Since many energy drinks pack 200 mg to 300 mg per can, they pose serious health risks for teens whose brains and bodies are still developing.

  • High doses of caffeine have been linked to abnormal heart rhythms, heart palpitations, and elevated blood pressure even in young, healthy individuals.

  • Excessive intake causes insomnia, jitters, anxiety, and severe sleep disruptions. In 2023, poison control centers saw a 20% spike in caffeine-related emergency calls from youth.

  • The risks are severely magnified for teens taking stimulant medications (e.g., ADHD drugs like Adderall or Ritalin) or asthma inhalers (albuterol), which can trigger dangerous stimulant stacking.

  • A recent study by Consumer Reports found that some energy drinks contain up to 16% more caffeine than their labels advertise, meaning teens may unintentionally ingest even higher doses than they calculate.
