What am I doing wrong with potatoes?

From the looks of that remaining tiny sprig, you likely had very severe pest problems. That looks like flea beetles damage to me, but it could also have been any number of other pests including the Colorado potato beetle which is very common pest to potatoes. Hard to say for certain.

Looking at the debris in the pot the sprigs did not reach any size to produce new potatoes and the combination of overwatering and pests probably led to their premature death.

If you are growing potatoes in a container it is very difficult to water down to the bottom so I used a plastic pipe (found as waste near a building site) with holes drilled in and push it to the bottom then water the top of the container and pour water down the tube. It worked for me. If worried about overwatering make sure there are drainage holes. Water logging causes the tubers to rot,
