Clinical Protocol for Preventing and Reversing Alzheimer’s, Cognitive Decline, and Brain Fog

In January 2024 Biogen discontinued the drug named Aduhelm. The FDA had granted the drug accelerated approval three years earlier, against the recommendation of its own advisory committee, and it performed the way it was designed to do. It cleared amyloid out of the brain. Lecanemab, its successor, cleared amyloid too, and did so inside a large and properly run trial. Christopher van Dyck and his colleagues reported the 18-month result in the New England Journal of Medicine: 1,795 participants, and on the Clinical Dementia Rating sum of boxes, an 18-point scale, a decline of 1.21 points in the treated group against 1.66 in the placebo group. The entire benefit of removing amyloid came to 0.45 of a point. Muir and colleagues, reviewing the question in Alzheimer’s & Dementia in 2024, put the smallest difference a patient or clinician would actually register at around a full point. Brain swelling or bleeding showed up on imaging in 12.6% of the people treated.

The plaque came out. The people carrying it declined anyway.

Dale Bredesen has spent 20 years arguing for a different reading of that result. Amyloid is a protective response. A brain manufactures amyloid as a defensive strategy against inflammation, withdrawn hormonal and trophic support, and toxic exposure. Clear the amyloid response, but the underlying mechanisms that drive plaque formation continue unabated. The analogy he originally conceived was: imagine a roof with 36 holes in it. A roofer patches one, stands back to admire the work, and the water keeps coming through the other 35. Bredesen identified 36 mechanisms that push a neuron toward destruction, and mitochondrial dysfunction sits underneath all of them. The 36 holes were never meant as a final inventory. In Bredesen’s later work, the roof has become less a list of 36 defects than a network of interacting metabolic, inflammatory, trophic, toxic, vascular and infectious pressures capable of pushing the brain toward degeneration.

A brain runs close to a quadrillion synapses, every one an energy-hungry junction that has to be powered, maintained and rebuilt. The mitochondria supplying that grid begin to fail, and the brain does what any system does when its power supply runs short: it shuts down the 36 mechanisms that least essential circuits first. The pathology runs for roughly 20 years before it earns a name. By the time a diagnosis is made, most of the decline being measured has already happened.
