When I Wondered About My Astigmatism

I have worn glasses for astigmatism for twenty years. Then I wondered whether autism and astigmatism are linked—and found a repeated correlation, several possible explanations, and no settled answer.

I’ve worn glasses for about twenty years.

Not because that is when my astigmatism began, but because that is when eye examinations and prescription lenses became something I could reasonably afford. Before then there were always more immediate demands on my income, and, if I’m honest, I wasn’t reading very much anyway. If the world was a little blurry, it was simply the world I knew.

Recently I found myself wondering whether autism and astigmatism occurred together more often than chance would predict.

Not because I had a theory.

Because I realised I had never asked the question.

The answer surprised me.

It turns out researchers have been asking it for quite some time. Across population studies, clinical ophthalmology, and systematic reviews, a remarkably consistent observation has emerged: autistic people are more likely to have astigmatism than the general population. One of the largest studies examined almost 900,000 adolescents and found the association remained even after accounting for other factors. A recent meta-analysis likewise concluded that astigmatism appears more common amongst autistic children than would ordinarily be expected.

The finding has been replicated.

The explanation has not.

Every paper I read eventually arrived at the same conclusion. More research is needed to understand why these conditions often appear together.

That distinction is worth preserving. It is easy to slide from “these things are associated” to “one explains the other,” but science depends upon resisting exactly that temptation. Observation comes first. Explanation follows only if the evidence eventually supports it.

What I also learned was that astigmatism is rather more interesting than I had appreciated.

Like many people, I had quietly understood it as a kind of blurry vision. In reality it is more specific than that. The eye does not focus light equally in every direction. Different orientations come into focus differently because the eye itself is not perfectly symmetrical. It is a geometrical property of the optical system rather than simply a reduction in sharpness.

That does not tell us anything about autism.

But it does remind us that perception begins before the brain has interpreted anything at all. The light entering the eye is already travelling through a physical structure with its own characteristics, imperfections, and history.

Whether that has anything to do with autism remains an open question.

For now, all we can honestly say is that autistic people appear more likely to have astigmatism than non-autistic people, and nobody yet knows why.

I find that oddly satisfying.

Not because uncertainty is an answer, but because it is a reminder of how science actually works. Sometimes the most important discovery is not solving a mystery. It is noticing, carefully enough, that there is one.
