Eighty-three men walked into a clinic in Catania carrying your exact problem.
Every one had erectile dysfunction. Every one had testosterone under the line. Mean age 55.3, and the youngest was 44.
Their doctors offered a choice of three doors.
The trial ran in Reproductive Biology and Endocrinology in July 2025.

Door one: tirzepatide, the weight-loss injection. 2.5mg a week for a month, then 5mg. Twenty-eight men took it.
Door two: no drug. Thirty men.
Door three: testosterone gel on the skin. 23mg to start, adjusted at day 14. Twenty-five men.
Every man in all three groups cut his daily calories by 20% and walked briskly for 20 minutes a day.
Eight weeks later they came back and answered the same five questions.
Those five questions run the whole study, and you can answer them tonight.
The test carries the name IIEF-5. Five questions about erections over the past six months, each scored, with a total between 5 and 25. Doctors read 5 to 7 as severe, 8 to 11 as moderate, 12 to 16 as mild to moderate, and 17 to 21 as mild.
Every man in this trial scored between 5 and 12 at the start. In the injection group, 18 of the 28 sat in the severe band.
Search the name, answer the five questions, write your total in your phone with today’s date.
Then find a tape measure and put it round your waist at the navel, standing, breathing out.
Every man here measured over 94cm to qualify. Their waists ran around 109cm, and in the no-drug group, 116cm.
Two numbers. Four minutes. Nobody in the house any wiser.
When did you last measure that waist with a tape rather than guessing from your trousers?
Here is what the five questions showed after eight weeks.
The men on tirzepatide improved their score more than the men taking no drug. Set against the men on testosterone gel, no difference appeared.
The injection matched the hormone on erections and beat it on weight, waist and every hormone measured.
Why this trial cannot hand you that as proof sits below.
