A Heart Surgeon’s Confession: It Was Never the Cholesterol

Dr. Philip Ovadia has stood over thousands of open chests. He is a board-certified cardiac surgeon who built a career repairing the damage of heart disease one operation at a time. And after all those hours on the operating table, he reached a conclusion that should stop every one of us in our tracks: most of the patients he operates on never had to be there.

That single realization is the engine behind his new book, Stay Off My Kitchen Table, the subject of his latest conversation with Robert Lufkin MD on the Health Longevity Secrets podcast. It is the sequel to his first book, Stay Off My Operating Table, and it goes deeper into the question that haunted him as a surgeon: if the standard advice worked, why did the operating rooms keep filling up?

Ovadia’s argument is not abstract. He lived it. He was once morbidly obese and prediabetic, over 100 pounds overweight, traveling down the exact same path as the patients he was cutting open. He had done everything “right” — eat less, move more, follow the food pyramid — and it wasn’t working. What finally changed things was encountering the work of science journalist Gary Taubes, which forced him to confront how much contrary evidence had been kept from him as a physician.

He lost nearly 100 pounds. More importantly, he rebuilt his understanding of what actually drives heart disease. The answer, he concluded, wasn’t cholesterol and it wasn’t a prescription. It was metabolic health.

One of the central messages of the new book — and one that represents a dramatic shift for a man who spent 30 years immersed in cardiology — is that meat is medicine. Ovadia argues that essential nutrients can only be obtained from meat, and that the nutrients found in plants are often poorly bioavailable to the human body. His co-author, Joshua Lisik, spent nearly a decade as a vegan, saw early gains, then watched his health unravel — a story Ovadia uses to illustrate that while you can pursue health on many diets, a nutrient-dense, meat-centered diet is far simpler to get right.

He is careful not to be dogmatic. He supports any dietary approach that helps people avoid and reverse chronic disease. But he insists on honesty: if your current plan isn’t producing measurable results, you need to understand why and be willing to change.

If red meat is so essential, how did it become public enemy number one? Ovadia traces it to a combination of poorly done epidemiologic science and the creep of political and economic interests into how that science was interpreted. Epidemiology is designed to generate hypotheses, not prove causation. Yet observational studies — riddled with the problem that people who eat more meat also historically smoked more and exercised less — were over-interpreted into dietary law. When better interventional trials later failed to show harm (and sometimes showed benefit), those results were largely ignored because they didn’t fit the accepted belief.

For the past 50 years we’ve been told to eat less meat — and we did. U.S. red meat consumption has fallen roughly 40% since the 1950s and 60s. Yet heart disease has gotten worse. That alone should tell us red meat isn’t the cause.

Ovadia is cautiously optimistic. He points to a changing national dialogue — government leaders now using words like “insulin resistance” and “seed oils,” a revised food pyramid, and a new federal website, realfood.gov, that emerged from a cross-party coalition. He sees hope in younger doctors who increasingly understand that simply treating disease is no longer enough; the future is prevention and reversal, which always circles back to diet and lifestyle.

Ovadia has walked away from employed surgical practice to run Ovadia Heart Health, a nationwide telehealth practice now licensed in all 50 states. What started as metabolic-focused heart care expanded into primary care because, as he treated the heart through diet and lifestyle, his patients’ diabetes, thyroid issues, and gut problems improved too. As Lufkin frames it: you don’t have three diseases — diabetes, heart disease, and gout. You have three symptoms of one disease, and that disease is metabolic dysfunction.

A key theme of Stay Off My Kitchen Table is that two patients on the identical diet can get wildly different results, because outcomes depend on what your gut can actually absorb. Ovadia explores gut health, bioavailability, and plant “anti-nutrients” that can block absorption. The conversation touches on a striking example: a recent meta-analysis suggesting oral NAD supplements don’t raise blood levels — a reminder that swallowing a nutrient is not the same as using it.

Asked about wearables, Ovadia’s top pick is unambiguous: the continuous glucose monitor (CGM). Seeing in real time how your blood sugar responds to food, sleep, activity, and stress is, in his view, the single most powerful tool for shifting how people understand diet and lifestyle. And because it can be monitored remotely, it’s tailor-made for telehealth.
