In an effort to help patients navigate our current economic challenging times CHM is offering Packages of visits (2 visit & 4 visit) with associated discounts.
You can purchase in advance and pay in full for 2 visits (saving 10%) or 4 visits (saving 15%) on total cost of visits. These Packages do NOT expire! Us them anytime. There is no contract. We ask our patients if they have not signed our Private Membership Association (PMA) agreement to do so to qualify.
For new patients to qualify they must complete their first two visits first.
This package offering is for visits with our Advanced Practitioners (APPs) only; that is our Nurse Practitioners and Physician Associates.
This package program excludes visits with Dr. Saleeby
