Feel, Heal, Reveal…

He texted me three words: I got the promotion.

And I cried.

Not because a lab value normalized. Not because a chronic disease marker stabilized. Not because I could write “pt stable, continue current treatment" in a chart note and move on to the next patient.

I cried because I knew what it took for him to get there.

I've been this patient's doctor for years- virtually, largely because he is isolated in all the ways that matter; isolated geographically, emotionally, spiritually. He is surrounded by people who don't believe in science, who don't believe in basic facts, who don’t discuss feelings, hopes and dreams, and who make him feel like caring about his own health is somehow “radical”. He has chronic diseases. His only daily companion is an aging pet. And for a long time, putting one foot in front of the other was the most difficult thing he literally did.

There was a stretch where I saw him weekly - not because his blood pressure was out of control, but because he was. Not in a dangerous way. In a human way. The kind of unraveling that happens when you feel unseen, unvalued, and unsupported for too long. So I showed up. I told him he wasn't a burden (many times. It was almost a sign off after each visit). I told him I valued him. I told him that longitudinal care, the kind where your doctor actually knows you, matters more than any single prescription I could write.

And then he got busy. Months went by. I worried. I made sure his meds were coordinated and chronic disease states were stable, and that nothing lapsed. But I didn't hear from him the way I usually did.

Then came the text.

I got the promotion.

Of course he did.

This is a medical win.

I know it doesn't look like one. There's no imaging to show, no before-and-after lab panel, no dramatic diagnosis story. But medicine - real medicine, the kind I practice-has never been just about disease. It's about the person carrying the disease. And sometimes the most important thing a doctor does is help that person believe they are worth fighting for.

Here's what I've learned in my years of doing this work:

Medical wins don't always look like "chronic disease stable, continue current treatment."

Sometimes a medical win looks like:

- Your patient finally talks to you from the safety of their bed because their nervous system is so fried they can't sit upright in a waiting room.

- Someone who has been paralyzed on their couch by an undiagnosed disorder finally gets into treatment and tells you, "I feel like I can live for the first time."

- A patient schedules the screening they've been avoiding for three years…the Pap smear, the Cologuard, the thing they were too scared or too overwhelmed to do.

- Someone advocates for themselves - at work, at home, at another doctor's office - because you taught them that their voice matters.

- A man who felt invisible gets a promotion, because somewhere along the way, he started to believe he deserved one.

That's medicine. 

That is feel, heal, reveal.

I'll be honest…and I don't usually say this publicly- things are truly HARD right now. For all of us. Doctors are exhausted. We are devalued. We are fighting for our patients while simultaneously fighting for our right to keep doing this work. We're watching trust in science erode in real time. We're not getting paid what we're worth - and many physicians are barely paying rent.

And yet.

We keep showing up. We keep texting back. We keep offering the discount we can't really afford because we know that continuity of care is worth more than any single visit fee.

…So maybe it's time we redefine what winning looks like in medicine. Not just for our patients, but for ourselves.

Because if you got your patient off the couch… that's a win.

If you got them to believe they're not a burden… that's a win.

If you got a text that said I got the promotion and you knew, you just knew, that your care was part of the reason - that is a win.

And we deserve to take it.

We NEED it….it nourishes us.

So I'm asking you. Whether you're a doctor, a patient, or someone who has ever been told they're too much or not enough...

It doesn't have to be a promotion. It doesn't have to be something “dramatic” or big. Maybe you got out of bed. Maybe you made the appointment you’ve been avoiding. Maybe you simply decided you weren't going to apologize for needing help.
