Because I do not evaluate based on a fixed method, there are things I need to know about the player to best help them improve. In the past, I have addressed most of the following concepts in some manner. I hope you have come away from the reading that there is no best way. Parents of junior players are great at asking, when given options, “which is the best way?” The answer is always it depends on the player. This questionnaire gives me a sense of what will work or not. For example, in question 8, I ask if you consider your motion to be rotary or linear. I know that certain postures and sources of motion lend themselves to certain techniques. If that does not match, we know this is an area of concern. The conflicts in your thinking are your biggest problem. There is no right or wrong answer to any of these individual questions; we are looking for conflicts.

Do you feel tension in your hands and forearms when you hold the club?
Do you tend to overread or underread putts?
Where do you tend to strike the ball on the putter’s face? Heel, center, or toe?
